※個資法同意條文 Personal Data Collection Agreement【請打勾】
以下是個資法同意條款內容 : *
一、共修期間：遵守佛光山之道風、堂規及接受領導、穿著修道服，配合團體生活。
二、同意條款：本人了解並同意佛光山寺及派下各單位得將本人所填寫的個人資料蒐集、
處理、使用在功德徵信、法會、活動及未來各項活動通知、帳務處理與功德主提報等作業。
且本人提供之本人以外其他個人資料，本人已以適當方式使該人等知悉及同意其個人資料會
提供予佛光山寺派下相關單位，進行蒐集、處理或利用。
1.During the course: I will abide by the monastic rules and regulations,
and I would follow the instructions and live in concordance with the community.
2.Agreement: I understand and agree that my personal information would be registered
for data collection and usage, and for future notifications on the activities of the
monastery. I also declare that I have notified and obtained the agreement of the people,
whose information was provided by me, that their information would be given to the
organizing unit of Fo Guang Shan for data collection and usage.
